What personal information do we collect?
Services & surveys. When requesting information, as appropriate, you may be asked to provide your name, email address, phone number or other details.
Job applications. Maple Health Group collects personal information when you apply for a position through the Maple Health Group website, or coordinate with one of our approved recruiters. This includes your name, mailing address, email address, telephone number, and background information required to apply for a job. Only Maple Health Group, or representatives acting on its behalf, will contact you using this information you have provided.
Conferences & events. The personal data collected varies across our site and may include information that you provide, such as your full name, organization name, mailing address, email address, telephone number, and background information required to apply for a job, attend an event, or to receive more information such as news and updates from Maple Health Group.

The processing of this request is free of charge, but we reserve the right, as allowed under EU GDPR Article 12 (5) and UK GDPR Article 12 (5), to charge an administrative fee under certain circumstances. Please note that we may refuse to act, as allowed under EU GDPR Article 12 (2) and 12 (5) and UK GDPR Article 12 (2) and 12 (5), on requests that are insufficiently substantiated, unfounded or excessive.
